Can this be placed (in a bag) in a high flow area or does it have to be used in a reactor?

This product is designed to be tumbled and will be most effective when used this way, but so long as you are using moderate amounts of media and you can spread it out in a media bag in a higher flow area, you can still benefit from it in this way.

How often must this be changed when used in a PhosBan Reactor?

It depends on the amount of phosphate you have in your tank and you add to your tank. Typically it needs to be replaced every 4-6 weeks but if you add a lot of food it could be much less and if you almost never feed it could be much longer. Best thing to do is test your phosphates, when they start to rise its time to change your GFO.
